[FB] Fixed Beat Mode

On the New Game screen, set Fixed Beat Mode to On and Double-Time to Off.

Non-Permadeath

On the New Game screen, select Permadeath Mode = Off.

Non-Mystery Mode

On the New Game screen, select Mystery Mode = Off.
